R-5 SSS summary — Answers to your questions

  • The name of the R-5 is "Employer Contributions Payment Form".

  • The purpose of the R-5 is to submit all employee and employer contributions to SSS.

  • To submit the form R-5, you must: 1. In Zipier,

    • Check the total amount due. (see form ➔)
    • Input the amount in words in the space above the signature.
    • Click "Sign" on the form.
    • Click "Submit" (below), then "Yes".
      1. Pay tax due. (see "Payment")
      2. Get the form and your receipt stamped.
      3. Take the stamped Payor's copy back for your records.
      4. Go back to Zipier.
    • On "To-Do", find the R-5.
    • Click the blue "Tax form waiting..." icon.
    • Click "Yep, it's sent".
      • Upload file or link that shows proof of submission.
  • The R-5 the due date is based on the 10th digit of the SSS number: 1 or 2 = 10th day of the month 3 or 4 = 15th day of the month 5 or 6 = 20th day of the month 7 or 8 = 25th day of the month 9 or 0 = last day of month

  • For the R-5, you must submit 1 copies.

  • The R-5 is to be submitted at your regional SSS office.
